Will you join us in this endeavor to meet the physical and spiritual needs of the people in Peru?

Each year a team of healthcare professionals and volunteers travel with Crossroads Outreach Medical Missions International (COMMI) to Iquitos in the Peruvian Amazon to offer free healthcare to pastors and their families during a pastoral conference.

Anyone is invited to join the team. Medical experience is not required.

TRIP COST:
The cost of trip is $2,800 and it covers airfare, meals, accommodations, local transportation, and travel insurance. Payments can be submitted as follows:

• Non-Refundable Deposit/Application Fee: $150, due at registration
• First Payment Installment: $1,350, due by 11/30/2025 ($1,500 if deposit not paid yet)
• Final Payment Installment: $1,300, due by 1/31/2025

TRIP REQUIREMENTS:
The following are required for each traveler:

1. Valid US passport (Passports must be valid for more than six months beyond the departure date to Peru. If your passport expires within this window, you will need to renew it before travel)
2. Copy of current medical or nursing license (if applicable)
3. Completed COMMI Trip Application
4. A signed Liability Release Waiver Form
5. All recommended vaccinations:
   • Measles
   • Polio
   • Tdap
   • Hepatitis A & B
   • Typhoid
   • Yellow Fever
